Oklahoma Prevention Needs Assessment Survey

Dear Parent or Guardian:
The Oklahoma Department of Mental Health and Substance Abuse Services is conducting an important survey on student behaviors and beliefs. The survey is called the Oklahoma Prevention Needs Assessment (OPNA) Survey. Your child’s class (6th, 8th, 10th, & 12th) is set to take this survey on December 5, 2023.
The purpose of the survey is to gather information needed to plan prevention and intervention programs in our community and schools. These programs will address problems of alcohol, tobacco, and other drug use, school dropout, delinquency, violence, mental health, and other issues in our schools and communities. Information will also help judge the effectiveness of current prevention and intervention efforts. The survey instrument is available at
https://oklahoma.gov/odmhsas/prevention/schools/opna.html. I also have a copy of the survey instrument at the school office if you wish to stop by and review it.
The survey is entirely anonymous and confidential. Students will not include their names or any other identifying information. All results from the survey will be presented only in group summary form.
The survey asks questions about behaviors students or students’ friends may or may not have done. It is important to remember the survey is anonymous, so no student’s response will ever be able to be connected with that student. For the survey, students will be asked to respond to questions such as “How wrong do you think it is for someone your age to pick a fight with someone at school?” “Which of the following activities for people your age are available in your community?” “On how many occasions (if any) have you used marijuana in the past 30 days?” “Do you share your thoughts and feelings with at least one of your parents/guardians?”
Your child’s participation in the survey is voluntary. Each child will have the option of skipping any question that he or she prefers not to answer. You may decline to have your child participate if you wish. Please remember this survey offers your child the opportunity to share his or her confidential opinions on very important issues being faced by our youth today. If you decline, your son or daughter will be allowed to read or participate in some other alternative activity while his or her classmates are taking the survey.

CLASSES WILL BE CANCELLED
TUESDAY NOVEMBER 7, 2023
In an abundance of honor and respect to the family of our beloved coach, Charles Kemp, including our Seminole Chieftain family, classes will be cancelled on Tuesday of next week (11/07/2023). A memorial service celebrating Coach Kemp’s life will be held at 10:30 a.m. that day, at the Seminole High School Gym. In regard to the school calendar, we ask that you think of this day in the same way we would think of a cancellation for a snow-day. The regular school schedule will resume on Wednesday November 8th. Thank you for your understanding during this time of shock, grief, and celebration of the life of Coach Charles Kemp.

Dear Community, Parents, and Students,
It is with profound sadness that we share the news of the passing of our beloved Coach Kemp. His unexpected death has left us all in a state of shock and grief. Coach Kemp was not just a mentor, but a guiding light who touched the lives of countless individuals in our community.
His dedication to his craft was unparalleled. He poured his heart and soul into coaching, always striving to bring out the best in each and every one of his students. His unwavering belief in the potential of every athlete, his ability to motivate and inspire, and his commitment to fostering a sense of unity and sportsmanship among the team were truly remarkable.
Beyond the wins and losses, Coach Kemp taught us invaluable life lessons. He instilled in us the values of perseverance, discipline, and teamwork. Coach Kemp showed us that success is not solely measured by trophies and accolades, but by the growth and development of each individual under guidance.
Coach Kemp’s impact extended far beyond the gym. He was a mentor, a confidant, and a friend to many. Coach Kemp took the time to listen, to offer advice, and to provide a shoulder to lean on during both triumphs and challenges. His genuine care and concern for his students went beyond the boundaries of the sport, creating a sense of family within the team.
As we mourn the loss of Coach Kemp, let us remember the legacy he leaves behind. Let us honor his memory by carrying forward the lessons he taught us, by embodying the values he instilled in us, and by supporting one another as a united team.
To his family, our community and students, we extend our deepest condolences. During this difficult time, let us come together to celebrate the life of Coach Kemp and to support one another through the healing process. Together, we will keep his spirit alive and ensure that the impact on our lives continues to inspire us for years to come.
